Florida is a Direct Access state. You may not need a physician referral or prescription for a physical therapy evaluation, so you can skip the cost of seeing a physician and skip the wait of trying to get into physical therapy, thus you can get treated faster. Medicare states you can be treated by a physical therapist for 28 days prior to needing a physician prescription. Medicare Advantage Plans require a signed plan of care prior to treatment, but can still be evaluated. Aetna, Florida Blue (BCBS), Humana, UHC, and Cigna currently do not require physical therapy referrals. If this all sounds complicated, it is. So feel free to message me with your specific case.
FPR will be accepting most major insurances. Currently we accept Medicare (no Advantage yet), Aetna, Tricare, Tricare East, Auto, ChampVA, Cigna, and then BSCS (Florida Blue) as an OON provider. The insurance section will be updated as FPR becomes a provider of additional insurances. If you are still unsure, feel free to message me.
This sounds strange, but with many private insurances requiring deductibles between $2500-$10000 (Florida Blue - Bronze) before insurance even starts covering you, sometimes cash-based pay is faster and cheaper. There are also many insurances which limit the number of visits you are allowed to attend in a rolling or annual basis.
The best response is whatever you feel comfortable in. But if we are being honest, some form of athletic attire or looser fitting clothing is ideal. Jeans are hard to move in compared to shorts or leggings, sandals are not as stable as tennis shoes. But think about what you are being treated for, can that area be easily assessed and treated in what you are wearing. If you have orthotics or braces consistently used, please bring those as well. If you are still unsure, feel free to send me a message and I will help guide you. Please bring a bottle of water with you, this facility does not have a water fountain.
The intake forms are the standard information I (and insurances) will need in order to treat you. Since I do not have access to your full medical record, providing me a general amount of information will help me determine the best (and safest) course of treatment. There are also outcome measures listed, these are ways I track objective progress over time as well as required by most insurances. The more information you can provide online, the less therapy time it will take up having you fill them out here.
